How We Cut HCG Hospitals' CPL by 40% Without Increasing Spend.
The fix wasn't in the ads. It was in the journey that happened after every click.
The problem
The campaigns weren't broken.
CTR was above 4%. Spend was pacing correctly.
But the patient journey — from first click to first human contact — was full of gaps no one was looking for.
The Context
HCG Hospitals is one of India's largest cancer care networks — a multi-city, multispeciality healthcare provider with dedicated oncology centres across India and an international oncology program serving patients from across the GCC and South Asia.
We managed two accounts simultaneously: India (multispeciality + oncology lead generation) and international (oncology patient acquisition). Budget: ₹50 lakhs per month. Targets: ₹1,000 CPL India, ₹5,000 CPL international, CTR 5%+.
The patient journey — before
The CX Gap We Found
What the dashboard didn't show.
The landing page took 3.2 seconds to load on mobile.
68% of all traffic was mobile. 3.2 seconds is the difference between a patient who books and a patient who calls the competitor down the road.
WhatsApp follow-up was taking 4 hours on average.
In healthcare, that's a lifetime. Patients who inquire are often anxious, comparing multiple hospitals in real time. A 4-hour gap meant they'd already decided elsewhere.
High-intent drop-offs received zero retargeting.
Patients who visited specialty pages and spent real time there — but didn't fill the form — were the highest-intent visitors in the entire funnel, and we were ignoring all of them.
The ad copy was identical across every oncology specialty.
A patient searching for breast cancer treatment saw the exact same message as someone searching for blood cancer treatment. Zero personalization by diagnosis.
“None of these problems were visible in the Google Ads dashboard. CPL looked ‘fine’ as an average. The gaps only appeared when we mapped the full CX journey.”
What We Fixed
We didn't touch the bids first. We fixed the journey first.
Compressed the landing page.
Mobile load time dropped from 3.2 seconds to 0.9 seconds — image compression, lazy loading, and removing render-blocking scripts.
Built same-hour WhatsApp automation.
Triggered the moment a lead form was submitted. Average response time: 12 minutes. Patients received a personalized message with the doctor's specialty and next steps before they had time to look elsewhere.
Created specialty-specific retargeting.
Separate audiences for each cancer type based on page-level intent. Visit the breast cancer page, see breast cancer specialist ads. No more, no less.
Rebuilt ad copy by specialty.
Separate ad groups per oncology type, written from the patient's actual emotional state — fear, hope, urgency for answers — not generic hospital messaging.
Only after fixing the journey did we touch the campaigns — reallocating budget to the highest-performing specialty-audience combinations and increasing bids during peak inquiry hours identified from the lead data.
The Results
Six metrics. Every one moved in the right direction.
The Lesson
In healthcare, every lead is a life. Precision isn't optional. And the CX gap is never where you think it is.
The instinct is to blame the campaigns. The truth is almost always in the journey — the moments between the first click and the first human contact. That's where patients are lost. That's where the cost hides.
Fix the journey first. The numbers follow.
